School-based Check
and Connect Program
Members will serve as Mentors in the school-based Check and Connect Program. Mentors work with a caseload of students who are identified as at risk of dropping out, forming trusting relationships with them, while monitoring their grades and behaviors. Mentors aim to help students build the skills they need to succeed in school. Positions as follows: note that half-time positions are 900 hours for 10 months (approximately 25-30 hours/week,) and full-time positions are 1700 for 12 months (approximately 40 hours/week) Cortez Middle School: 1 half-time, 10-month position Dolores Middle & High School: 1 full-time, 12-month position Escalante Middle School: 1 full-time, 12-month position Mancos Middle & High School: 1 half-time,10-month position Montezuma-Cortez High: 1 half-time, 10-month position Park Elementary (Durango): 1 half-time, 10-month position

Montelores County
School Community Youth Collaborative/Montelores Early Childhood Council 1 full-time member will serve as the Youth Programs Assistant, and will facilitate the High School Leadership Montezuma and Youth Leadership Council programs. Member will work with the Heart and Soul Community Initiative, ensuring that youth voice is part of the process. 1 full-time member will serve as the Community Organizer, with time split between SCYC and MECC. This member will be instrumental in the planning, implementation, and evaluation of the Early Childhood Celebration and the Teen Maze. Member will recruit and manage volunteers, and will also implement Family night activities. This position will be filled by a returning member.
Dove Creek Students Taking action against Underage Drinking (STUD) 1 full-time member will serve as a Community Prevention Organizer, and will help coordinate the student-led STUD group, and will plan and implement regular and ongoing programs, projects and activities, including Peer Mentoring, Kick Butts Day, Relay Under the Stars, Honoring Graduates, Summer Fun Activities, and more. This position will be filled by a returning member.
Durango
(serves the whole region)
Durango Adult Education Center 1 full-timemember will serve as the SWConnect Project Coordinator, an internet-based information hub for southwest Colorado. Member will map assets, write content articles, develop a youth page, manage volunteers, and train community members. This position will be filled by a returning member.
Durango and
La Plata County
La Plata Youth Services 1 full-time member will serve as a Community Outreach Coordinator, and will coordinate community partners, organizations and individuals to create linkages designed to increase and improve opportunities for youth referred to La Plata Youth Services. LPYS serves risk youth and families with the goal of diverting them from future or further court involvement. This is a 12-month position.

San Juan Basin Health Dept’s CHP 1 full-time member will serve as a Community Prevention Organizer with the Celebrating Community Norms project. Member will ensure community collaboration, assist with community outreach and training events, and help implement a responsible parenting social media campaign. Member will participate in information dissemination along with research and continual evaluation of program. This is a 12-month position.

Fort Lewis College, Student Wellness Initiatives 1 half-time member will serve as a Student Wellness Initiatives Prevention Assistant with WellPAC, a student-led organization that uses peer educators to promote healthy student behavior at FLC. Member will help plan and implement new student orientation activities, and will work to help strengthen the current 50’ smoking perimeter campus-wide policy. This is a 12-month position.
Ignacio SUCAP Youth Services 1 full-time member will serve as a Youth Services Specialist, and will work with staff to implement youth leadership programs in the community of Ignacio, Colorado. Member is primarily responsible for staffing the Teen Center after school Monday-Friday. Member will help implement Project Venture, an experiential education and outdoor education program designed for Native American youth.
Pagosa Springs Pagosa Springs Nurturing Center 1 Half-Time member will serve as a Youth and Family Advocate, and will participate in, lead groups and mentor individuals in a variety of programs for youth, emerging adults and their families, including the Nurturing Parents Program, Nurturing Teens Program, See It. Stop It. Program, Growing Space, and Alateen. This is a 12-month position.
Silverton Silverton Youth Center 1 full-time member will serve as a Youth Center Coordinator, and will assist with the operation of a high-quality, year-round youth center, chaperoning during regular drop-in hours, and planning and implementing special events. Member will develop collaborations with other community organization, and will manage the YAKs (Youth Advisory Council.) This position will be filled by a returning member.
